Personalized cancer vaccine shows effect in major trial
A vaccine built specifically around each patient’s tumor works. That sounds almost too good to be true, but a large randomized trial now provides the strongest evidence yet.
For years, researchers have been developing an idea: a vaccine that does not prevent cancer but responds to an existing one. The genetic profile of an individual patient’s tumor is mapped, and a vaccine is produced that trains the immune system to recognize and attack those specific tumor cells.
Researchers from Merck and Moderna presented results from a randomized late-stage clinical trial. This is the first time a personalized mRNA cancer vaccine has demonstrated evidence of effectiveness in a controlled study of this scale. The full outcome data had not yet been completely published at the time of writing.
How the vaccine works
The vaccine uses mRNA technology, the same approach used in COVID-19 vaccines. The mRNA gives immune cells instructions to recognize proteins found only on the patient’s tumor cells. These are called neoantigens: protein fragments that arise from the specific mutations in that person’s tumor. In theory, the immune system learns to target the individual patient’s cancer.
For longevity research, cancer is a central concern. After cardiovascular disease, cancer is the leading cause of death in people over 65. Cancer risk rises sharply with age, in part because the immune system ages and cells accumulate more mutations. A vaccine that keeps the immune system attuned to tumor antigens fits directly into that framework.
What remains unknown
Personalized vaccines are expensive and time-consuming to produce. Each patient requires a separate manufacturing process. How scalable this will be, and for which cancer types it works best, remains unclear. The published results have also not yet been fully peer-reviewed. Caution in interpretation remains appropriate until the full data are available.
