A gene-edited pig kidney made way for a human one

Two patients first received a kidney from a genetically modified pig and then, months later, a human kidney. Both transplants succeeded. This shows that a temporary animal organ can keep a patient alive until a human organ becomes available.

Xenotransplantation, the transplanting of organs between species, has been a concept for decades but has proved difficult to achieve. One major concern was whether removing a pig kidney and later giving the patient a human kidney would cause the body to produce antibodies that would interfere with the human transplant. Based on primate studies, scientists at biotech company eGenesis did not expect this to happen.

The researchers now have confirmation from two patients. After the pig kidney was removed, each patient returned to dialysis for several months until a human organ became available. Both subsequently underwent successful human kidney transplants. The pig kidney thus served as a temporary bridge.

Why this matters for the organ shortage

Each year, people die on organ transplant waiting lists. Genetically modified pigs are seen as a potential solution: their organs are comparable in size to human organs and can be modified through gene editing to reduce rejection. The question was always whether an animal organ would block the path to a human one. These are the first two cases showing that it need not.

The transition was not seamless. Both patients had to wait months on dialysis after the pig kidney was removed. But the period without dialysis, during which the pig kidney functioned, represented a meaningful improvement in quality of life.

Early stage, many open questions

These are two case reports, not a randomised trial. Long-term safety data, the optimal patient population, and the lasting effects of animal organs on the immune system remain largely unknown. For longevity applications, where organ function in later life is central, xenotransplantation remains a distant prospect for now.
