Does long-term antibiotic use weaken your immune system?

Antibiotics disrupt the gut microbiota in ways that indirectly weaken your immune system; use them only when truly necessary and discuss with your doctor whether a course can genuinely not be avoided.

Antibiotics reduce the diversity of the gut microbiota and alter its composition. This is reasonably well documented across multiple reviews. Gut bacteria are not merely passengers: they actively train your immune system, keep the gut wall intact, and protect against dangerous invaders.

One of the most concrete consequences is a reduction in so-called colonisation resistance. This is the ability of your own gut bacteria to keep harmful bacteria out, such as Clostridioides difficile or multidrug-resistant hospital bacteria. When antibiotics wipe out the normal flora, those invaders gain more opportunity to establish themselves. The production of antimicrobial peptides, small proteins the body uses to fend off bacteria, also declines after antibiotic use. Antibiotics targeting gram-positive bacteria can additionally reduce the production of butyrate. Butyrate supports the gut wall and modulates the immune system. Less butyrate potentially means a weaker barrier.

The link between antibiotic use and broader immune problems, such as autoimmune diseases and chronic inflammation, is for the time being associative. That is to say: a correlation has been found, but cause and effect have not yet been definitively established. Reviews suggest that a less resilient microbiota could contribute to the rise of autoimmune diseases in wealthy countries, but this is a hypothesis, not a proven mechanism.

There is also a practical point for cancer patients receiving immunotherapy: a disrupted gut microbiota is associated with a poorer response to so-called checkpoint inhibitors. Antibiotics may have an indirect influence on this. This link too is associative, but it is one reason why physicians in that context are taking an increasingly critical look at antibiotic use.

Recovery of the microbiota after antibiotics is variable: in some people it takes months, while in others the composition is altered for an extended period. According to the reviews examined, there is no single supplement or diet that reliably resolves this.
