What do we know about berberine patches?

Preliminary evidence

Berberine patches show interesting results in laboratory and animal research for wounds, eczema, psoriasis and joint complaints, but no tests in humans have yet been conducted, so no clinical conclusions can currently be drawn from them.

Berberine patches come in many forms: hydrogels, nanofibre membranes, microneedle patches and vesicle-based gels. What they have in common is that they aim to deliver berberine (a plant-derived alkaloid) directly through the skin, so that the substance acts at the site of the complaint without passing through the intestines. In laboratory tests, a cellulose-based hydrogel patch delivers more than 100 micrograms of berberine per square centimetre through artificial skin in 24 hours, and does so without causing damage to skin cells.

For wound care, laboratory experiments show that a berberine nanofibre membrane absorbs blood more rapidly and promotes clotting better than ordinary gauze, and that it has broad antibacterial activity. A carboxymethylcellulose-based hydrogel reduced oxidative stress and excessive cell growth that can impair chronic wound healing in cell culture by approximately 25% less unwanted cell growth.

For skin conditions such as psoriasis and eczema, the signals come from mouse studies. A berberine hydrogel reduced redness, crusting and characteristic inflammatory markers in mice with artificially induced eczema, and improved the skin barrier. For psoriasis, a combination of berberine and mangiferin in a self-forming gel was tested: it worked better than either substance alone, partly because it appeared to inhibit a specific immune process (in which immune cells release harmful nets). A microneedle system inspired by traditional cupping delivered berberine through the skin more efficiently and reduced inflammation in laboratory and animal experiments.

For joint complaints, researchers tested a microneedle patch containing berberine and sinomenine in mice with rheumatoid arthritis: the combination reduced inflammation and unwanted blood vessel formation in the joints more effectively than either substance alone. Another gel containing berberine in lipid vesicles reduced cartilage breakdown in animals with arthritis without causing liver or kidney damage. These are interesting safety signals, but they have not yet been confirmed in humans.

All of these findings come exclusively from laboratory and animal research. No clinical trials in humans have yet been conducted. Whether the patches are safe and effective in humans, and which type works best for which condition, is therefore unknown. This is an active area of research, but it is far too early for practical use.
