What does your klotho level tell you about how fast you are ageing?
Your klotho level demonstrably declines with age and is associated with greater disease risk, but whether measuring it is already useful as a personal ageing clock has not been proven. No standardised cut-off values for clinical practice exist yet.
Klotho declines measurably with age. That is a consistent pattern, observed across more than 65 studies in healthy people and even more pronounced in people with kidney or metabolic disease. Low levels are associated with a higher risk of chronic kidney disease, atherosclerosis, diabetes, Alzheimer's disease and cancer. These are associations, however, not proven cause and effect.
What those low levels mean in practice for mortality has been investigated in more than 2,600 frail, middle-aged to older adults. Higher klotho levels were associated with a hazard ratio of 0.68 for all-cause mortality and 0.48 for cardiovascular mortality. That is a substantial difference, but again: an association from an observational study, not proof that klotho itself is the cause.
There is also an important catch: the relationship between klotho and mortality is U-shaped. Not only very low levels, but also very high levels were associated with greater mortality in frail older adults. The conclusion 'the more klotho, the better' is therefore incorrect. A specific threshold for what is optimal has not yet been established for routine practice.
Animal research paints an impressive picture. Mice without a functioning klotho gene develop something that closely resembles accelerated ageing and live much shorter lives. Mice that are genetically engineered to produce more klotho live on average around 30% longer. Whether this also applies to humans has not been demonstrated. Klotho also inhibits certain inflammatory pathways that play a role in the low-grade chronic inflammation that is characteristic of ageing, but that mechanism has largely been described in cell and animal research.
Your klotho level can already be measured in blood, urine or cerebrospinal fluid. However, there are no standardised measurement methods or officially established cut-off values for use in general practice. A number on paper therefore means very little at this point without that clinical context. Therapies to raise klotho from the outside, such as protein injections or gene therapy, are still at an early stage and have not been approved.
