Brain changes in Alzheimer start years before plaques
Amyloid plaques are considered the earliest visible sign of Alzheimer’s on a brain scan. But the brain begins changing at least seven years before that, according to nearly two decades of brain imaging in healthy older adults.
PET scans can detect amyloid buildup in the brain and have long been the gold standard for early Alzheimer’s detection. But what if measurable brain changes occur before those scans show anything at all?
The researchers followed healthy older adults with repeated brain scans over nearly twenty years. They detected signals of brain change an average of seven years before the first detectable amyloid plaques appeared. This suggests the disease process is already underway while current diagnostic tools still show nothing unusual.
What those early changes involve
The study does not fully characterise which specific changes occur, but they likely involve subtle shifts in brain structure or activity that only become apparent through repeated measurements over long periods. Single scans miss these signals entirely, making early detection with current methods difficult.
Consequences for prevention
If the disease process begins seven years earlier than previously assumed, the therapeutic window is larger than thought. But it also raises a question: what causes those very first changes, and can they be slowed before amyloid starts accumulating?
From a longevity perspective, this finding matters. The boundary between ‘healthy’ and ‘early disease’ sits further back in time than assumed. That makes preventive interventions in midlife potentially more meaningful. The study is observational and long-term, but the causal mechanisms behind those early changes have not yet been established.
