Cyclarity clears phase 1 trial for 7KC drug
Most treatments for cardiovascular disease work by lowering cholesterol or dampening inflammation. Cyclarity is taking a different route: targeting a toxic cholesterol byproduct buried deep within plaque itself.
Cyclarity is developing UDP-003, a drug aimed at 7-ketocholesterol (7KC). This is a cholesterol breakdown product that accumulates inside macrophages, the repair cells that try to clear plaques from blood vessel walls. When macrophages take on too much 7KC, they turn into so-called foam cells: bloated cells that end up feeding further plaque growth rather than stopping it.
UDP-003 is designed to remove 7KC from macrophages, allowing those cells to get back to doing their job. The researchers report that the phase 1 trial, conducted in Australia in partnership with Monash University, has been completed successfully. That means the drug proved safe and well tolerated in healthy participants.
A different angle than statins
Statins lower LDL cholesterol, cutting off some of the raw material that plaques are built from. UDP-003 works further downstream: it goes after damage that has already been done. That makes it potentially useful as a complement to existing treatments, not a replacement for them.
The logic behind the approach is that plaques do not grow solely because of cholesterol supply. They also persist because foam cells are no longer able to clear the plaque effectively. By restoring macrophages to a functional state, it may be possible to actively break down hardened plaque.
Phase 1 is done, but the real work starts now
A successful phase 1 trial demonstrates safety, not efficacy. That question has to be answered in phase 2, where the drug will be tested in people who actually have cardiovascular disease. It also remains unclear what doses will be needed to produce a measurable effect on plaques.
Even so, this is a meaningful step forward. 7KC has long been considered a difficult target. A phase 1 result pointing to safety opens the door to a range of clinical questions that simply could not be asked before.
