Fasting is everywhere in longevity circles, but what does it genuinely deliver over the long run? We break down where the evidence is solid, where it still wobbles, and what a sensible choice looks like if healthy aging is your goal.
Fasting promises a lot: a longer life, younger cells, a healthier metabolism. Scientists are far more excited about the mechanisms than about hard outcomes in humans. Eating less works in animals; in humans it is more complicated
The starting point is clear: caloric restriction consistently and substantially extends the lives of laboratory animals. For healthy, normal-weight humans, that direct evidence simply does not exist yet. Research mapping the biological effects of caloric restriction in people does show favorable changes in aging markers, insulin sensitivity, and inflammatory markers. Intermittent fasting: roughly on par with simply eating less
Intermittent fasting, most commonly in a 16:8 or 5:2 format, does biologically interesting things: it lowers insulin, supports cellular repair processes, and improves weight and blood pressure in some people. The catch is that head-to-head comparisons with ordinary caloric restriction produce no clear winner. For weight loss and metabolic improvement, the two strategies land close together. The fasting-mimicking diet: real effects, limited independent research
The fasting-mimicking diet (FMD), best known through the commercial ProLon program, involves eating at sharply reduced calories for five days each month. A controlled study of 301 participants showed measurable improvements in weight, fat mass, and blood pressure. The caveat is that virtually all the research comes from a group with a financial stake in the product. The results are therefore interesting but not yet definitive. Black coffee fits just fine inside your fasting window
A question that comes up constantly: can you drink coffee while fasting? Black coffee contains virtually no calories and does not break your fasting window. Nutritional guidelines covering intermittent fasting explicitly recommend it as a suitable drink during a fast. Fasting works better on top of a solid nutritional foundation
Fasting is not a substitute for what you eat during the hours you do eat. Of all dietary patterns, the Mediterranean diet has the strongest and most consistent evidence in large human populations, particularly for cardiovascular disease and cognitive health in later life. Legumes, a core component of that pattern, are linked to a lower risk of death in research covering more than 1.7 million people. A predominantly plant-based eating pattern reinforces that effect, though the benefit lies in the quality of what you eat, not in a "vegetarian" label.
The most sustainable approach, then, combines a moderate time-restricted eating pattern with genuinely high-quality food: plenty of legumes, vegetables, healthy fats, and as little ultra-processed food as possible. No fasting schedule compensates for a consistently poor diet.
