Platelet-rich plasma outlasts cortisone for spinal pain

Cortisone injections are a standard treatment for back pain caused by spinal narrowing. But a new study in dogs suggests that platelet-rich plasma may provide more durable relief. The condition has direct parallels in aging humans, making the comparison worth noting.

Lumbosacral stenosis is a condition in which the spinal canal narrows at the lower back, causing pain and reduced mobility. It affects dogs commonly and has a close human equivalent in degenerative lumbar stenosis, a major driver of pain and disability in older adults. Conservative injection therapies are widely used because they carry lower procedural risk than surgery.

A retrospective study compared three treatments in 55 dogs: injections of platelet-rich plasma (PRP), epidural corticosteroid injections (methylprednisolone), and a mesotherapy protocol combining local anesthetics and anti-inflammatory agents. The researchers measured pain severity and mobility at multiple time points up to 120 days after treatment.

PRP holds its effect longest

All three treatments produced significant pain improvements through the first 90 days. But only the PRP group maintained those improvements through the final measurement at 120 days. The difference in pain scores at that point was statistically significant (p = 0.03). The cortisone and mesotherapy groups showed more regression after 90 days.

PRP works by concentrating the body’s own growth factors and injecting them at the site of damage to support tissue repair. Cortisone suppresses inflammation directly but temporarily. The longer durability of PRP may reflect an underlying tissue repair effect rather than just symptom suppression.

What this means for joint health in aging

This is an animal study and the translation to human spinal conditions requires separate clinical research. PRP is already used in people for joint-related conditions, with mixed evidence in the human literature. From a longevity standpoint, the finding is worth tracking: if PRP provides more sustained relief than corticosteroids in degenerative spinal conditions, it could become relevant for preserving mobility in older adults. That said, the retrospective design and animal population limit how far these conclusions can be extended.
