Age-related eye disease driven by retinal inflammation

Age-related macular degeneration is the leading cause of central vision loss in older adults. What exactly drives it has long been unclear. A new review study points to a specific inflammatory mechanism inside the retina.

The retina is part of the central nervous system. Like the brain, it is vulnerable to the chronic, low-grade inflammation that increases with age. In macular degeneration (AMD), the central part of the retina, the macula, gradually deteriorates. Early-stage disease has few treatment options; advanced forms respond to some therapies, but not consistently.

The researchers map how the NLRP3 inflammasome, a protein complex in the innate immune response, plays a key role in AMD. The NLRP3 inflammasome acts as an alarm system inside cells: it detects stress signals and triggers an inflammatory response. In the aging retina, this alarm becomes chronically activated, even without acute infection or injury.

Cell type determines the outcome

A particularly striking aspect of this review is that the consequences of NLRP3 activation differ by retinal cell type. In some cells, it amplifies inflammation in early AMD. In others, it promotes the loss of retinal tissue in advanced dry AMD, or drives blood vessel growth in the wet form. This makes a universal therapy difficult: a drug that helps in one disease stage may be harmful in another.

The researchers also highlight the risks of existing anti-inflammatory drugs, which also block the protective side of the immune response. Finding an agent that suppresses only the harmful NLRP3 activation, without disrupting useful immune functions, is a major challenge in AMD drug development.

Aging as the common thread

The review connects AMD to broader aging processes. Chronic inflammation, also called inflammaging, is a hallmark of aging tissue throughout the body. The retina is no exception. From a longevity perspective, AMD is not only an eye disease but also a manifestation of systemic aging. Whether targeting inflammaging could slow AMD progression remains an open research question.
