Does creatine work better with carbohydrates?

Insulin appears to activate the transporter that pumps creatine from the blood into muscle cells. Because carbohydrates trigger insulin release, the combination sounds logical: more insulin, more creatine in the muscle.

Yet it is not that straightforward. Taking a large carbohydrate meal together with creatine can actually slow absorption from the gut. Insulin helps on the muscle side, but intestinal absorption appears to suffer as a result. These two opposing effects make the overall picture unclear.

Both findings come from the same limited body of research, and the actual magnitude of each effect has not been measured. There are no concrete figures on how much more or less creatine ultimately ends up in the muscle. What the combination means for sports performance or muscle strength over the longer term cannot be determined from these data.

From a practical standpoint: there is no reason to combine creatine specifically with a large amount of carbohydrates in the hope of a clear additional benefit. Creatine works perfectly well without them. If you do take it with a meal, an ordinary mixed meal (with some carbohydrates and protein) is fine, but a large sugar load purely to 'boost' absorption is not yet supported by evidence.
