Does creatine work better with carbohydrates?
Carbohydrates likely increase the uptake of creatine into the muscles, but whether this also leads to better performance is not demonstrated by randomised research. Add carbohydrates if you want to load, but do not automatically count on extra training results.
A small study using muscle biopsies (16 participants) showed that creatine combined with 100 g of sucrose raised phosphocreatine levels in the muscles further than creatine alone. Phosphocreatine rose from 87.6 to 106.2 mmol per kilogram of dry weight in the group that also received alpha-lipoic acid, while the other groups lagged behind. Carbohydrates without alpha-lipoic acid already offered an advantage over creatine alone, but the difference was greatest in the combination group.
The explanation lies in insulin: carbohydrates stimulate its production, and insulin helps transport creatine into the muscle cell more quickly. Yet it is not that straightforward. Large meals high in carbohydrates can actually slow the absorption of creatine from the intestine. The net effect is therefore unknown and has not been quantified in the available studies.
But whether that increased muscle uptake actually leads to better performance is the second question. A randomised study compared creatine alone with creatine plus 70 g of carbohydrates and a third group with creatine plus cinnamon. All three groups improved on anaerobic peak power and muscle strength, but there was no statistically significant difference between the groups. Extra carbohydrates therefore produced no measurable performance difference.
A narrative review also mentions potential benefits for accelerating muscle glycogen replenishment after exercise when combining creatine with carbohydrates, but draws no firm conclusions and calls for further research. The timing of creatine intake in relation to food intake has in general been little studied, and the available reviews provide no clear-cut recommendation.
Pay attention to the safety margins as well. Doses above 6 grams of creatine per day are considered a therapeutic dosage. There are indications of possible effects on insulin regulation and kidney function over the long term, although formal safety studies are lacking. If you use more than those 6 grams, discuss this with your doctor.
