Does Ozempic change your brain?

Ozempic demonstrably changes how your brain responds to food and hunger, which is the core driver of the weight loss. Whether it also protects against brain diseases such as Alzheimer's has not yet been proven in humans.

Ozempic (and similar drugs containing the same active ingredient, semaglutide) influences brain regions that regulate appetite and reward. In a randomised study, researchers found that a GLP-1 receptor agonist reduced activity in brain regions such as the insula, amygdala and orbitofrontal cortex, areas that respond to the sight of food. That effect largely disappeared once the GLP-1 receptor was blocked, which makes it plausible that it genuinely works through that receptor.

The weight loss people experience with Ozempic is now well understood as a brain effect. A recent review confirms that GLP-1 receptor agonists work primarily through the central nervous system: they activate receptors in multiple locations in the brain, reducing hunger and making you feel full sooner, sometimes even before you have started eating. Animal research and human brain samples show that a specific area in the hypothalamus plays a key role in this. Beyond appetite, there are also indications that the reward system becomes quieter: the brain responds less strongly to calorie-dense food, and a systematic analysis of human studies found lower hunger levels and calorie intake. Whether this also affects addictive behaviour remains uncertain.

For neurodegenerative conditions such as Alzheimer's and Parkinson's disease, the early signals are positive but still very preliminary. In mice with an Alzheimer's model, semaglutide improved memory and reduced harmful protein accumulations. Similar effects were seen in laboratory models of human brain tissue. Observational research and early clinical studies show promising signals, but whether this genuinely has a protective effect in people over the long term has not yet been demonstrated. Long-term data on both efficacy and safety are lacking, and how well the various drugs cross the blood-brain barrier differs from one drug to another.

In the study on pre-ingestive satiety and in the systematic analysis, financial ties to Novo Nordisk (the maker of Ozempic) were disclosed by several authors. That does not mean the findings are incorrect, but it is a reason to read those particular results with a little more caution.
