Estrogen use linked to lower Alzheimer’s risk

Women who used estrogen-only hormone therapy later in life were 39 percent less likely to be diagnosed with dementia. They also showed fewer Alzheimer’s hallmarks in their brains after death. The numbers are striking for such a contested therapy.

A large study of more than 21,000 participants examined what happens in the brains of women who used estrogen-only hormone therapy in later life. The researchers looked both at clinical dementia diagnoses and at brain tissue examined post-mortem. Women who had used estrogen showed 35 percent fewer signs of Alzheimer’s disease, including deposits of amyloid protein and tangles of tau protein. Blood-based biomarker tests also suggested less amyloid accumulation in hormone therapy users.

This is an observational study. The researchers cannot prove that estrogen caused the protection observed. Women who use hormone therapy differ from non-users in other ways that may influence the results.

Timing and type of therapy matter

Earlier research produced mixed findings, partly because many studies examined combination therapy (estrogen plus progestogen). This study focused specifically on estrogen alone, typically prescribed to women who have had a hysterectomy. That makes the comparison cleaner, though other confounding factors may still be present.

Timing likely matters too. Evidence suggests estrogen is most protective when started early in menopause, during a so-called critical window. Whether that was the case in this study cannot be determined from the available summary.

What this means in practice

Hormone therapy has been debated for decades, notably after the 2002 Women’s Health Initiative study highlighted risks for heart disease and breast cancer. Current clinical thinking is nuanced: for healthy women around menopause, benefits may outweigh risks depending on age, therapy type, and individual risk factors.

This study adds a new dimension to that debate but does not change guidelines for now. The authors suggest estrogen is a potentially neuroprotective agent, but emphasise that further research is needed before firm conclusions can be drawn. From a longevity perspective, the findings are notable because they point to a biological window during which hormonal protection of the brain may be possible.
