Healthy midlife blood vessels add twelve dementia-free years
People with no vascular risk factors at age 55 live more than twelve years longer without dementia than those who have three. That is the main finding of a large US cohort study with over twelve thousand participants.
Dementia is not purely a brain disease. The state of the cardiovascular system has a decisive influence on how the brain ages. Smaller vessels disappear with age. Larger vessels sustain damage over time. Reduced physical fitness also cuts blood supply to the brain. The result is a slow, cumulative loss of support to brain tissue.
Damaged blood vessels also allow unwanted cells and molecules to leak into the brain, triggering what researchers call chronic low-grade inflammation (persistent, low-level inflammatory activity). Blood vessel walls become a source of inflammatory signals themselves. High blood pressure worsens this by causing repeated small ruptures in brain vessels, each one leaving a trace of damage.
Twelve extra years without dementia
The study, drawing on the Atherosclerosis Risk in Communities cohort, followed participants for a median of 26 years. Those with zero risk factors at 55 (no diabetes, no hypertension, no current smoking) remained dementia-free for an average of 30.1 years. Those with all three risk factors: only 17.5 years. The gap was 12.6 years.
The hazard of developing dementia was nearly 2.7 times higher in people with all three risk factors. Strikingly, the hazard of dying without dementia was more than five times higher. This means many people with poor vascular health die before dementia has a chance to emerge.
What this means for prevention
The findings align with a growing view in longevity research: the cardiovascular system and the brain do not age independently. Maintaining vascular health in midlife appears to be one of the strongest protective factors against cognitive decline in later life. The researchers are clear that these are associations, not proven causal relationships. Further research is needed to confirm whether targeted interventions translate to clinical benefit.
Still, the scale of the association is substantial and consistent: vascular risk factors at midlife appear to shape how many healthy years follow.
