Metformin against aging: what the evidence now shows
Metformin has been the most widely prescribed diabetes drug for decades. Now there is growing interest in whether it can slow aging in people without diabetes. The answer is more nuanced than many enthusiasts hope.
Metformin belongs to a class of compounds called biguanides and lowers blood sugar via several mechanisms. It activates AMPK, a protein that acts as an energy sensor inside cells. This leads to suppression of mTOR (a growth signalling pathway that, when overactive, accelerates aging), increased production of new mitochondria (the cell’s energy generators), and more cellular cleanup (the process by which cells remove damaged components). The review authors also highlight effects on gut microbiota as a possible additional mechanism.
In people with diabetes or metabolic syndrome, the evidence is stronger. The MeMeMe trial showed that 1,700 mg of metformin per day reduced the risk of developing diabetes by 80 to 92 percent in this high-risk group. That is substantial. But whether the same applies to healthy people without a metabolic disorder remains an open question.
The big trial is still pending
The TAME study (Targeting Aging with Metformin) was designed to answer exactly that question. With 3,000 participants aged 65 to 79 and a four-year follow-up, it is the most ambitious test of metformin as an aging-slowing agent. Yet nearly a decade after its launch, no results have been published. There are indications that funding difficulties have contributed to the delay.
Meanwhile, the animal data are mixed. In some mouse strains, metformin extends lifespan; in others, the effect is minimal or absent. Compared to rapamycin, another widely discussed compound, metformin looks less impressive for lifespan extension in animal studies.
Caution warranted
The review authors are broadly positive but acknowledge that the quality of existing human evidence is limited. For people with diabetes or metabolic syndrome, metformin is a proven drug. As a geroprotector for healthy older adults, it remains a promising but unproven hypothesis. That distinction deserves more prominence in popular longevity discussions.
